Primary Care  NPs is please to announce that we have partnered with Denver Area Assisted Living Communities in providing in-house primary care.  We bring the doctor’s office to you.. Dr. Elizabeth Kelts leads our team of Nurse Practitioners and we have more than 35 years of experience caring for the elderly. Our team takes pride in the medical care we provide. We work closely with families in managing patient care and navigating all the difficulties of healthcare. We value our partnership with the families and facility staff, understanding that it takes a team effort to coordinate the best health practice for our patients.

  • We specialize in elder care and assisted living/retirement primary care.

  • Laboratory services in-house

  • Xray and diagnostics in-house

  • We make referrals for our patients to see specialists

  • Management of medications to include working closely with pharmacies/medication refills

  • Chronic Disease Management

  • Telemedicine—but prefer in-person visits even during a pandemic.

  •  24/7 urgent phone access to assist staff/families with changes of conditions.

  • We work closely with multiple hospice, home health, and outpatient therapy agencies.

  • We work with insurance companies to navigate Prior Authorizations for procedures and special medications.  

  • Our providers are in the building 2-3 times per week

  • Management of Chronic and acute conditions

  • Health Promotion and Care Coordination to our patients

  •  Triage nursing Monday through Friday during business

  • Electronic medical record patient portals

  • Electronic prescription refills that are fast and easy

 

NP Commitment to Primary Care According to AANP:

Almost all NPs (89 percent) are prepared in a primary care focus, e.g., adult, family, gerontological, pediatric or women’s health. The family NP (FNP) focus is the most prevalent category (see below). Regardless of their population focus, primary care NPs are prepared to fulfill the definition of primary care across settings, including the provision of care at first contact for undifferentiated conditions, ongoing management of acute and chronic conditions, health promotion and care coordination.

Summary

For more than 45 years, NPs have provided patient-centered health care to a broad range of populations. The American Association of Nurse Practitioners’ (AANP) vision for the future calls for high-quality health care for all by the patient’s provider of choice. There is a critical need for all primary care providers to practice to the full extent of their scope of practice. As our nation faces the future challenges of an aging population, rising health care costs and the growing burden of chronic disease, NPs bring strength to the health care workforce and maximize our available potential to address these challenges.
